You will need to bring the following:

• INSURANCE INFORMATION
• DRIVER'S LICENSE OR PICTURE IDENTIFICATION

Bring the following if you have:

• X-RAYS
• CT SCANS
• MRI SCANS
• ATTORNEY INFORMATION
  

If you have difficulty obtaining these records, please call us and we will assist you.

Without records, it may be necessary to reschedule your appointment.
